Hospice is not a place; it is a way of caring for patients with a life limiting illness. The focus of care is on comfort, quality of life and personal dignity. Hospice works with patients and their families to make each day the best it can be. Care is provided anywhere a patient may live; in their own home, family member's home, an independent care setting or an assisted living facility, or in a skilled nursing facility.

Hospice cares for the whole person by focusing on improving physical comfort and providing emotional and spiritual care. A Hospice team works with the patient and family to develop an individualized plan of care to meet their specific needs as they are the center of everything we do.

The Hospice Team Approach

A dedicated group of specially trained professionals, the Hospice team starts with the patient - who is facing a life-limiting illness - working with them and their family to meet their needs. These individuals come from different professional backgrounds and work together to address care holistically. The team consists of physicians, nurses, medical social workers, spiritual caregivers, bereavement support people specially trained volunteers, physical, occupational and speech therapists, pharmacists, dietary counselors and other professionals.
