Outpatient Medical Services include:

Geriatric Consultations

  • Comprehensive assessments measures mobility and functional status, mental status examinations, depression screenings, evaluation of common geriatric problems (e.g., fall risks, incontinence and polypharmacy).
  • Recommendations regarding prevention and management of common geriatric syndromes and functional impairments.
  • Development of comprehensive plan of care with primary care physician, family members and the patient themselves.
  • Coordination of care with primary care physician.
  • Ongoing management of geriatric syndromes and functional impairments.

Geriatric Consultations are made through primary care physician referral, by calling 860.523.3801.

Outpatient Blood Transfusions

  • The Hospital at Hebrew Senior Care offers outpatient blood transfusions per physician referral
  • Patients receive attentive care in a healing, comforting and quiet environment, conveniently located on the first floor of The Hospital at Hebrew Senior Care. Parking is free and located just outside of the Hospital entrance
  • Family members are welcome to accompany patients to their appointments and may relax in our family area
  • The Nurse Manager can meet with family members or patients to answer questions about the transfusion process and any other services
  • Transfusions are scheduled quickly to meet the patient’s specific needs, including evenings and weekends – 24 hours per day, seven days per week
  • The Hospital at Hebrew Senior Care partners with Clinical Laboratory Partners (a Hartford HealthCare provider), so patients may go to any Clinical Lab drawing site to have the initial blood drawn. Patients’ physicians can call 860.218.2300 to schedule a transfusion appointment

Outpatient Infusions (including Prolia and Reclast)

The Hospital at Hebrew Senior Care offers the infusions to residents and the community on an outpatient basis.  

If you are interested in any of the above treatments or services, please print out the paperwork (if applicable) and give to your Primary Care Provider to fill out.
