  • A team approach to health care, characterized by core interdisciplinary teams consisting of one or more of the following, as required on a case-by-case basis: medical doctor, nurses, psychiatrist, psychiatric nurses, psychiatric social worker, pharmacist, physical, occupational and recreational therapists, rehabilitation specialists, social worker, dietician, neuropsychologists, and pastoral care providers.
  • Individualized treatment plans and daily assessments of patient progress and adjustment of care plans.
  • Discharge planning throughout the entire plan of care.

The Difference Between The Hospital at Hebrew Health Care and The Hebrew Home

Hebrew Home is a long-term skilled nursing facility providing health services to residents who are medically stable but can no longer live in the community. The Hospital at Hebrew Health Care serves elderly patients experiencing an exacerbation of symptoms of chronic conditions and patients with one or more behavioral needs. Patients may be referred from emergency rooms, skilled nursing facilities, assisted living facilities or community providers.

The Hospital at Hebrew Health Care is a certified Medicare and Medicaid provider. Coverage from third party payors may also be available.

How We're Different From "Regular" Hospitals

Traditional hospitals provide high quality care to the non-elderly, and to the elderly with straightforward health needs. The Hospital at Hebrew Health Care's board-certified geriatricians are specially trained in internal medicine and are aware of the specialized needs of the hospitalized elderly. We understand geriatric syndromes including nutritional needs and screen for medical conditions specific to older adults that have an impact on recuperation.
