Dr. Ava Pannullo

Dr. Ava Pannullo is the Medical Director for the Hebrew Home and Rehabilitation Center and for the The Hospital at Hebrew Health Care.

Dr. Pannullo oversees the medical department and treatment of all Hebrew Home and Rehabilitation Center residents and all patients at The Hospital at Hebrew Health Care . In addition to seeing her own patients, Dr. Pannullo is also directly involved in supervising the medical care of residents and patients who are assigned to other Hebrew Health Care physicians, including the physicians at Connecticut Geriatric Specialty Group.

In addition, Dr. Pannullo is an Assistant Professor of Medicine at the UCONN School of Medicine, and is Board Certified in Internal Medicine and Geriatrics.

Dr. Pannullo earned a Bachelor of Science degree from Vanderbilt University, and graduated from the UMDNJ. She also completed her residency at the UMDNJ, and earned a fellowship in Geriatric Medicine from the University of Connecticut Travelers Center on Aging.

Dr. Henry Schneiderman

As Vice President of Medical Services for Hebrew Health Care, Dr. Henry Schneiderman is ultimately responsible for all medical practices and policies throughout the organization, including Connecticut Geriatric Specialty Group. He is also responsible for maintaining Hebrew Health Care’s reputation for providing exceptional care to older adults.

Also, as Physician-in-Chief at Hebrew Home and Rehabilitation Center, Dr. Schneiderman sees patients daily, and oversees a staff of several other full-time physicians, as well as the Hebrew Home and Rehabilitation Center nursing staff.

In addition to his responsibilities at Hebrew Health Care, Dr. Schneiderman holds the rank of full Professor of Medicine (Geriatrics) at the University of Connecticut Health Center and continues to teach at UCONN and at Hebrew Health Care. He was named “Clinician of the Year” for 2005 by the American Geriatrics Society.

Dr. Schneiderman has an undergraduate degree from Yale, and graduated from Tufts Medical School. He completed residencies in both anatomic pathology and internal medicine at Rush and Loyola in Chicago. He is Board Certified in Anatomical Pathology, Internal Medicine, and Geriatrics.

Dr. Maureen Dana

Dr. Maureen Dana brings a decade of geriatric medicine experience to Connecticut Geriatric Specialty Group. She has been practicing geriatric medicine since 1999 at Hebrew Health Care’s Hebrew Home and Rehabilitation Center, and has served as an Assistant Professor of Medicine at the University of Connecticut School of Medicine over that same time period.

Dr. Dana’s education and training include a Bachelor of Arts in Biology from Johns Hopkins University, a Doctorate of Medicine from SUNY at Buffalo School of Medicine, Residency in Internal Medicine from the University of Connecticut, and a Fellowship in Geriatric Medicine at the University of Connecticut.

Dr. Dana has presented on dementia and osteoarthritis, and has won awards for excellence in ambulatory care and geriatric teaching.

Dr. Olusegun Apoeso

Dr. Olusegun Apoeso has practiced geriatric medicine for more than 10 years, and has treated hundreds of patients in both inpatient and outpatient settings.

Dr. Apoeso is an expert in pain management, and has received specialized training, conducted research, and published on this topic. He also specializes in infection control, and developed the infection control protocols for Hebrew Health Care.

In addition to his role as a Staff Physician at Connecticut Geriatric Specialty Group, Dr. Apoeso is an Assistant Professor of Medicine at the University of Connecticut School of Medicine and is the Medical Director of Outpatient Services for Hebrew Health Care, the leading geriatric health services provider in Greater Hartford.

After receiving his undergraduate and medical degrees in Nigeria, Dr. Apoeso trained at the UCONN Internal Medicine Residency Program and as a Fellow in Geriatric Medicine at Brookdale Department of Geriatrics and Adult Development at the Mount Sinai School of Medicine in New York City.

Dr. Sharon Farber

Dr. Sharon Farber’s experience in geriatric medicine includes three years as an attending physician at Hebrew Home and Rehabilitation Center, the skilled nursing affiliate of Hebrew Health Care in West Hartford.

Prior to beginning her professional career in geriatric medicine, Dr. Farber earned a bachelors degree from the University of California at San Diego, a Masters of Public Health from San Diego State University, and her medical degree from Sackler School of Medicine – New York State American Brand, in Tel Aviv, Israel.

Dr. Farber served as Chief Resident in the Department of Family Medicine from July 2003 to June 2004 at University Hospital at Stony Brook in Stony Brook, New York. She earned a Fellowship in Geriatric Medicine at University Hospital at Stony Brook, and is Board Certified in Family Practice and Geriatrics.

Elizabeth Esstman, APRN

Beth Esstman is a Geriatric Nurse Practitioner for the Hebrew Home and Rehabilitation Center. She has been a nurse practitioner for 9 years, and has been working at the Hebrew Home for 7 years, providing healthcare to patients on the long term care units, the sub-acute unit, and the Hebrew Medical Center. She currently practices primarily on the sub-acute unit caring for geriatric patients there.

In addition, she is a clinical instructor for Yale University, and oversees nurse practitioner students from Yale doing their clinical training at the Hebrew Home.

Beth received her RN and Masters in Nursing from Yale University and is certified as a Geriatric Nurse Practitioner by the American Nurses Credentialing Center (ANCC). She has been published in The Consultant.

Heidi Haller, APRN

Heidi Haller, APRN, provides healthcare to long-term care residents and short-term rehabilitation patients at Hebrew Health Care. In collaboration with the attending physicians, Ms. Haller manages the residents/patients chronic and acute medical conditions.

Ms. Haller has been an APRN at Hebrew Health Care for two years. Prior to her current position she was a staff nurse at The Hospital at Hebrew Health Care. While pursuing her Masters in Nursing, she worked as a staff nurse at Massachusetts General Hospital.

Ms. Haller has a BS from the University of Massachusetts, Amherst and received her RN and MSN from the MGH Institute of Health Professions in Boston, MA. Ms. Haller is certified as an Adult Nurse Practitioner through the American Nurses Credentialing Center and has been published in The Consultant. Over the past year she has mentored Nurse Practitioner students from both University of Connecticut and Yale University.

Lisa Halpern, APRN

Lisa Halpern, APRN, provides healthcare to long-term care residents at Hebrew Health Care. In collaboration with the attending physicians, Ms. Halpern manages the residents' chronic and acute medical conditions.

Ms. Halpern has been a member of the Hebrew Health Care team since June 2006. She graduated from Princeton University with a BA in Comparative Literature and received her RN and MSN from the Yale School of Nursing. Ms. Halpern maintains board certification in geriatrics from the American Nurses Credentialing Center and has been published in Advance for Nurses and the American Journal of Nursing.
