Our licensed Home and Health Services Agency (ALSA) provides on-site health care services for our assisted living community, Hoffman SummerWood which is located at 160 Simsbury Road, West Hartford, CT. Health services include nursing assessments, management of medications, coordination of personal care assistance, and companion services. The Home and Health Services Agency provides services under the direction of the member’s primary care physician. We can assist our clients with coordination of complex care plans by navigating community and health resources.
